Patient Services

Request Medical Records

You can obtain your medical records for services provided at any WhidbeyHealth location by completing the following steps:

  • Download the Release of Information form.
  • Please return your signed and completed form to the Medical Records Department (Health Information Management) located on the ground floor of the Medical Center. If you are unable to bring in the physical form, you can fax it to (360) 678-7623.
  • There may be a fee for the release of medical records.
  • The turn-around time to receive your medical records is approximately two weeks.

If you would like help filling out your Release of Information form, please call Health Information Management at (360) 678-7656 x6402 or (360) 321-7656 x6402.

If you need copies of Diagnostic Imaging films to be made/sent to your provider, please call the Diagnostic Imaging film room directly at (360) 678-5004.

Scheduling an Appointment

Central Scheduling is your first point of contact when scheduling an appointment with Stress Testing and select services noted below.

  • Diagnostic Imaging

    Contact Central Scheduling for appointments including:

    • CT and MRI scans
    • Mammogram
    • Ultrasound
    • Nuclear Medicine
    • DEXA Scan
    • Echo
    • Fluoroscopy
    • X-ray

    Diagnostic Imaging services are available at the main Medical Center campus. For your convenience, routine screening mammograms and X-rays may also be scheduled at our Goldie Street and Clinton clinic locations.

    Routine mammogram screenings do not require a provider order. All other exams do require a provider order and may need prior authorization from insurance payers.

  • Nutritional Services

    Appointments for nutritional services at the Medical Center and the Walk-In Care clinic in Oak Harbor are available for:

    • Medical Nutrition Therapy
    • Diabetic Nutrition counseling

    All Nutritional Services require an order from your provider and may require prior authorization from your insurance company. Provider orders can be faxed directly to Central Scheduling at (360) 678-7652.

  • Respiratory Therapy

    Appointments for Respiratory Therapy services are scheduled at the Medical Center campus and all appointments require a provider order. They may also require prior authorization from your insurance company.
